What is the Nurse at Home Program?

Our program delivers comprehensive in-home clinical services, including patient counselling, medication administration, blood glucose and blood pressure monitoring, opioid agonist treatment, foot care, and adherence evaluations. We also provide fall risk assessments to identify potential hazards in the home and offer practical solutions to help prevent falls and ensure a safer living environment.

What is the registration process?

Staff that are experts in the program include our manager, Marcus. Marcus is always in the store and is the go to person for this program. In addition, other staff members that know the program well include Jai and Rachel.

Is there a scheduled time and frequency for nursing visits?

The nurse is scheduled to arrive once daily every morning. However, the schedule is dependent on location and patient preferences. We strive to arrive based on patient’s expectations and we keep a consistent daily arrival time so that our patients can plan their schedules accordingly.

Are there any additional costs?

This service is free for you. All prescriptions must be paid for by the patient, or health benefits through your work or Pharmacare (Province of B.C.).

Can medications be filled and delivered from any pharmacy of choice?

To ensure continuity of care and proper monitoring, all medications related to the program are dispensed through Pharmasave Hamilton Village.

What cities are serviced?

We currently serve select cities across the lower mainland: Richmond, New Westminster, Delta, Surrey, Vancouver and Burnaby.
